Well one interesting point that has been made that I read in an old psychology essay in here is about how difficult it is to diagnose something as being a mental illness, because it's a judgement call. If the patient claims to be Napoleon, for it to be a mental illness, the clinical observer would need to disagree with the patient. If patient presents with a strong fear that the communists are after him, in order for this to be diagnosed as a mental issue, the observer has to think that the communists are not after him. If the observer agrees with him (say this was done in the 60s over here), then he would be considered normal. So it's a societal judgement call. The author of this essay was trying to discredit the existence of mental illnesses though, which I disagree with.
